Description The UCDH Health Management and Education (HME) Department serves to improve the health of patients through empowerment, self-management support and resource provision. The program aims to provide an effective and cost-efficient system of delivering and managing care for patients with chronic illnesses or at risk due to health behaviors HME is comprised of three primary programs: Post-Acute Discharge Call Program, Care Management, and Education.

Qualifications REQUIRED -Bachelor's Degree in health education, health science, or health administration with concentration in education, or similar. -Minimum of 3 years of experience teaching classes for adult patients regarding wellness, health promotion, or disease specific topics. -Minimum of 2 years managing staff providing educational content to adults -Demonstrated ability to develop, design, and implement a variety of education modalities including web-based, on-demand, or other electronic platforms -Strong knowledge of medical field, health promotion and/or education programs. -Demonstrated management, leadership, and conflict resolution skills -Strong presentation, written, and verbal communication skills. -Strong skills in decision-making, analysis, critical thinking, project management and organization leadership -Must be able to travel to the UCDMC campus, UCDH clinics and community locations throughout the greater Sacramento region
PREFERRED -Master's Degree or industry licensure/certification in a relevant area

Responsibilities The Health Education Manager leads the Education program within HME, providing strategic guidance for the program and advancing curriculum delivery modalities. This position's primary responsibility will be to manage and direct the activities of education initiatives including patient education, population health improvement for patients with chronic illnesses, and quality improvement efforts.
