IJP is hiring for a full-time, grant-funded Community Health Educator 2 to provide non-medical HIV case management, outreach, and navigation at all Juniper locations to current and former inmates with HIV as well as youth living with HIV.
The Community Health Educator 2 will partner with the clinical team to coordinate services, applications, referrals, and access to housing and social services. S/he will be responsible for data entry and tracking related to these services and facilitating regular evaluation of the program and reports to funders.
The Community Health Educator 2 must be organized and punctual, a team player, and have the skill to identify regional resources that can benefit people living with HIV, youth, and former inmates. A successful candidate will have the great attention to detail needed to manage caseloads and reporting and will also possess the communication skills needed to liaise with staff, partner agencies, and clients.
The Inova Juniper Program is Northern Virginia's largest provider of HIV/AIDS care. We offer all aspects of primary medical care and mental health, substance abuse, and case management services to HIV positive individuals. The Program also houses an HIV prevention program and a healthcare worker education program.
